Street Level Health improves the health and well being of uninsured, underinsured and underserved immigrant and refugee communities in the Bay Area.

About This Cause

Street Level Health Project is a 501c3 non-profit serving the greater Oakland, Alameda County, Bay Area, that is home to a growing community of immigrants and refugees, experiencing risk factors that pose psychological and physical public health threats associated to extreme generational poverty; emotional trauma; and the informal labor markets that they are forced to access due to immigration status. Increasingly, we provide a safety-net to low-income families in an area where census data reports 48% of households live below federal poverty levels (42%) remain uninsured. As Alameda County's remaining Day Laborer center, also known to many as "Proyecto de Salud Para Todos”, our center is a critical entry point to the health care and social services system for a disproportionately impacted population where many are needs Limited English Proficient adults. Clients triaged, are 93% foreign-born, with over 33 languages represented on an annual basis. Street Level Health Project reduces incidence of malnutrition and chronic illness through free same day drop-in urgent care that prioritizes concurrent medical, mental health, nutrition/herbalist, health navigation, and health enrollment services. Through our Whole Person Care model we also help Day Laborers plagued by unpredictable employment, navigate decreased wages that limit access to basic needs. Through twice weekly street outreach, up to 85 times per year, Community Health Workers collaborate with a Licensed Nutritionist to encourage low-to-no-income Day Laborers access a nutritious breakfast; health education; and a free lunch service concurrent with clinic hours. In 2018 alone, our off-site outreach, on-site case management, and health center served over 14,615 community members. In order to support long-term health and employment sustainability, we link a County Health Insurance Technician to our Community Health Workers to help clients navigate complex billing issues. Our ability to respond to the unique needs of Medi-Cal ineligible immigrants increased protective health factors for approximately 206 unduplicated clients over 327 mental health consultations; and reduced preventable hospital admissions for 810 unduplicated clients over 1,398 clinic visits, annually. We are also proud to announce that through our partnership with local medical schools, hospitals, and clinics, our Community Health Training program, receives and trains around 30 volunteers, per year, in the health center, who all go onto medical school, once they complete their hours.
