Mission Statement
Vision: A healthier Kershaw County, where individuals and communities are empowered to take charge of their own health and well-being. Mission ("How to achieve the Vision"): We will provide medical care and connect resources to achieve a healthier Kershaw County. The Community Medical Clinic provides free medical care for our neighbors who are underserved or uninsured in Kershaw County. Today CMC plays an increasingly broad role as the leader of a collaborative effort to improve population health throughout the county. The clinic itself, the hub of our operations, serves as the medical home to 800 county residents, resulting in about 3,600 patient visits annually
About This Cause
*The Community Medical Clinic (CMC) is more than a family medical practice. The clinic provides help with resources in four main areas, which include nurse practitioner visits, health coaching, medication assistance, and economic stability. These services are provided at no charge to the Kershaw County low-income residents that meet the eligibility criteria. We aim to provide high-quality services that embrace all (not just clinical) facets of an individual’s life, by working together in a team-based approach. In addition to our nurse practitioners, CMC also offers chiropractic services, counseling, diabetes education, physical therapy, and breast cancer and cervical cancer screenings. We can also make referrals to providers who partner with us. These specialties include surgery, urology, nephrology, pulmonology, dermatology, breast specialist, gastroenterology, endocrinology, cardiac surgery, pathology, radiology and emergency dental. *CMC is honored to be a trusted steward of a program funded through the Duke Endowment, AccessKershaw. This program connects people in need of medical, dental, vision, and mental healthcare with local providers and resources. More than 7,000 people have received the care they needed thanks to this program. The transitional care program (Access Kershaw) functions as a link between the uninsured and local health professionals including medical, dental, vision, and mental health care. This program was founded in 2010 with a grant from The Duke Endowment. Since 2015, CMC has been the grantee. Access Kershaw staff help individuals navigate the maze of healthcare, as well as connect people with a variety of support they may need outside of medical care, such as food, housing, clothing, and other crisis resources. Our transitional care program has reduced the number of uninsured patients being readmitted to KershawHealth. There has been a 25% decrease in ER visits for non-emergency purposes that can be directly attributed to the work of CMC. One ER visit for a patient without insurance usually costs more than $1600. A hospital admission can be upwards of $28,000. It’s important for us to provide preventative care for those who don’t have access to medical care. *CMC operates a School-Based Health Center- it's like a minute clinic within the Kershaw County High Schools and one middle school. We have expanded our reach to provide care to more students by using tele-health. This was possible through a partnership with the Telehealth Alliance through MUSC. *The LiveWell Kershaw Coalition (LWK), an initiative led by the Community Medical Clinic of Kershaw County and funded through The Duke Endowment’s Healthy People, Healthy Carolinas initiative, and the Institute for Healthcare Improvement as a SCALE 2.0 Community. The purpose of founding LWK was to use data to guide action aimed at addressing the health issues faced by the residents of Kershaw County. Today, LWK works to foster collaboration between citizens of all ages, working in partnership with public and private sector organizations and schools to increase community awareness of health issues. The goal is to improve access to quality healthcare and encourage the people of Kershaw County to take ownership of the health of the whole county, acting together to reduce chronic disease and obesity among the members of the community.
