Mission Statement
The mission of the North by Northeast Community Health Center is to improve health outcomes in a medically under-served community by offering health screening and basic medical services at no cost. Priority is given to low-income individuals without health insurance living in the surrounding inner N/NE neighborhoods.
About This Cause
The idea of establishing a community clinic for uninsured adults in North and Northeast Portland was born in 2006 to address the health care needs of Hurricane Katrina survivors who had been relocated to Portland. It soon became obvious that lack of health care was a problem not only for those displaced by the hurricane. Many long-time neighborhood residents were also going without needed medical care because they lacked health insurance and couldn't afford a doctor's visit, lab tests, medications or preventive care. Our volunteers provide patient visits at no cost during twice-weekly evening clinics, for walk-ins on Thursdays and follow-up appointments on Mondays. In addition to visits with a clinician, we provide prescription and over-the-counter medications, lab services, referrals for other health and social service needs and referrals for specialty care, all at no cost to our patients. Our focus is on the greatest need among our patient population – the management of common chronic health conditions such as diabetes, hypertension, asthma and depression, and the prevention of costly and potentially devastating complications. Our Quality Assurance process enables us to measure the success of our programs through patient outcomes and is essential to our ability to provide quality care. In addition to our weekly evening clinics, we provide daytime visits with our nurse Patient Care Manager for blood pressure checks and education about medication adherence, health concerns and healthy lifestyle choices. This year we developed a program of wellness activities that includes a walking group, a vegetable garden that provides produce to patients along with opportunities to learn about gardening and healthy eating, and a Patient Wellness council to direct these activities.
