About This Cause
Central Texas Palliative Care Associates dba Austin Palliative Care (APC) is a subsidiary of Hospice Austin. APC is a 501(c)3 corporation that was created in 2001. Access to Palliative Care The community-based palliative care program increases patient access to palliative care. Most members of our community have little—if any—access to palliative care in their place of residence prior to hospice. Austin Palliative Care (APC) will deliver community-based palliative care to adults with a life-limiting illness in Travis, Williamson, Bastrop and Hays counties. Support High-Quality Palliative Care/End-of-Life Discussions With an eye toward improving health outcomes, APC provides expertise in symptom management, education on disease processes, support for caregivers and families in a holistic manner (addressing physical, psychosocial, behavioral, and spiritual suffering), coordination of care with other providers, and navigation through the healthcare system. To help ensure that patients’ wishes are upheld, APC's interdisciplinary team will lead conversations about end-of-life care. While these conversations will likely be challenging, particularly with patients who may still be seeking curative treatments, the clinical staff on the team have extensive experience helping families navigate end-of-life issues, including advance care planning and goals of care discussions. Connection to Specialty Care Central Texans face barriers to specialty care. Outside of hospice or hospital-based services, there are a limited number of physicians with expertise in palliative medicine, an emerging specialty. Also, there can be significant communication challenges when providers do not coordinate care plans for patients with a life-limiting illness. APC providers employ a consultative, or co-managed approach so that patients with a life-limiting illness retain their primary care provider and/or specialist(s) while participating in the program. With a comprehensive, team-based approach, including expert medical care and psychosocial support, APC addresses complex needs, and helps streamline and improve care.
