About This Cause
Founded in 1992 by Webb Chapel United Methodist Church and Chapel Hill United Methodist Church, Woven Health Clinic began as a one-night-per-week volunteer-run clinic. Due to high community demand, we expanded, moving to Dallas Medical Center in 1995, and later opening a second location at Carrollton Regional Medical Center in 2021. Over the past 30 years, we have served over 30,000 patients, ensuring access to comprehensive healthcare services that focus on wellness and disease prevention. Texas faces a healthcare crisis, with 18.4% of adults uninsured, the highest rate in the U.S. In Dallas County alone, 21% of adults lack health insurance, often delaying care due to cost. Nearly one in three Texans report skipping medical care due to affordability, leading to worsened chronic conditions like diabetes and hypertension. Texas ranks last in healthcare access, and one in four women lack access to preventive care, contributing to late-stage diagnoses. At Woven Health Clinic, we bridge this gap by offering high-quality, preventive, and primary care services to the uninsured and underinsured. We go beyond treating illness—we focus on wellness, prevention, and addressing social determinants of health. Specialized Programs at Woven Health Clinic EMPOWER Health Program The EMPOWER Health Program is designed to help patients actively manage chronic conditions through personalized education and preventive care. We provide comprehensive health screenings, home blood pressure monitors, and self-care education tailored to each patient’s needs. This program also includes one-on-one health coaching sessions that empower patients to make informed decisions about their health. By equipping them with the right tools and knowledge, we are seeing a significant improvement in disease management, medication adherence, and overall well-being. Women’s Health Program Women’s healthcare is a cornerstone of our services, addressing disparities in access to essential preventive care. Our Women’s Health Program offers annual wellness exams, breast and cervical cancer screenings, and reproductive health services. We also provide education on maternal health, menopause, and chronic disease prevention specific to women’s health needs. With one in four women in Texas lacking access to preventive care, our program ensures early detection and treatment of potentially life-threatening conditions. Heart Health Champions Heart disease remains the leading cause of death in the U.S., yet many cases are preventable with proper management. Through our partnership with the American Heart Association, the Heart Health Champions program is dedicated to improving cardiovascular health. We provide personalized treatment plans, lifestyle counseling, and continuous monitoring to help patients control hypertension and reduce the risk of heart disease. As a result, 81% of our hypertensive patients have achieved controlled blood pressure, far exceeding the national average of 54%. Mind Matters Wellness Program Mental health is an integral part of overall well-being, yet many individuals lack access to necessary care. Our Mind Matters Wellness Program bridges this gap by offering individual therapy, psychiatric consultations, and mental health screenings. We integrate mental health into primary care, allowing patients to receive holistic treatment that addresses both their physical and emotional health. Over the past six months, 69% of our patients with depression have reported symptom remission, demonstrating the success of this program in improving mental health outcomes. At Woven Health Clinic, we take a holistic approach to healthcare, ensuring that every patient receives the support they need to achieve optimal health. We address not only medical concerns but also social and economic barriers that impact well-being.
