Mission Statement
Momentum is the largest private nonprofit provider of adult mental health services in Santa Clara County. Our program and services for individuals living with a mental illness are comprehensive and include a prevention and early intervention program, adult and transitional age youth (14 to 24) out-patient services, employment services, day rehabilitation services, residential treatment, supported housing and a volunteer program. In addition, Momentum works to reduce and eliminate the stigma that surrounds mental illness and mental health care through a community relations program that promotes education, diversity and collaboration. Momentum for Mental Health's mission is to help individuals achieve mental and emotional health, discover and reach their potential, and fully participate in life.
About This Cause
Momentum helps more than 4,000 people a year reclaim their lives by offering a wide array of comprehensive services: Transitional-Age Youth Services (TAY): Comprehensive outpatient mental health services, group therapy, and skill development activities for young adults with a mental illness diagnosis (aged 14 to 24). R.E.A.C.H (Raising Early Awareness and Creating Hope): Prevention and early intervention programs aim to prevent the onset of psychosis in young people who are experiencing symptoms of a mental illness (aged 10 to 25). Outpatient Services: Case management, therapy groups, and rehabilitation services for adults with a serious mental illness. Medical Services: Comprehensive psychiatric diagnosis and treatment and access to primary care at one of our sites. Full Service Partnership (FSP): Oupatient services are provided at two sites and are based on a recovery-oriented social rehabilitation model. Highly interactive, intensive assistance is provided for people facing complex challenges. Residential Treatment: Four Social Rehabilitation Residential facilities provide group and individual treatment to clients who need intense support during difficult times in lieu of hospitalization. Board & Care: Our licensed facilities provides psychiatric services, medication management, and therapy groups. Supported Housing: In-home support is offered to 106 individuals who are preparing to live independently. Rehabilitation Services: Programs provide counseling, skill development, peer support and life management tools. Employment Services: Participants receive help with building their resume, interviewing and obtaining employment.
