About This Cause
Patient Care Intervention Center (PCIC) is a Houston-based non-profit building a new approach to improving the health of communities. Our work puts people’s values and goals at the center, because we believe health systems should align with the context of a patient’s life. We utilize far-reaching data sets from across our partnerships with health systems, government, and social services agencies. Our technology, care coordination, and population health research teams all rely on this data to keep the patient’s values at the front, while simultaneously providing a better understanding of social determinants of health –– the largest predictor of health outcomes. In doing so, we’re challenging everyone whose work touches on the health of individuals to see healthcare as a partnership, between hospitals, clinics, government, business, social services, and communities — all centered on the values and barriers of patients. Our values-based model helps us understand who our patients really are by documenting their personal values, aspirations and what really matters to them most in life. For example, we may find that they value independence, they’re driven by family, and that they’re looking to stabilize an existing health condition. We then try to get a better picture of how they live, so we can put their health in context of their life. It’s one’s diet, physical activities, job stress, relationship with substances, the quality of their friendships, the safety of the air they breathe and the water they drink. In seeing the full picture of our patients’ lives, and understanding what drives them, we are ensuring that their healthcare isn’t just being streamlined, but that we’re making real, impactful change in their lives that promotes autonomy and wellness. At PCIC, everything we do helps providers gain a deeper understanding of our patients. Our technology platform encourages care workers to ask the questions that get at a patient’s values, aspirations, and barriers. We also ensure that everyone who provides care is included on our platform. This means not just hospitals and clinics, but also food banks, housing providers, and other social service agencies. Our system aggregates data across all our community partners, which allows us to create a directory that maps resources across the community. It also allows us to make more effective referrals and connect patients to the help they really need. This approach not only improves health outcomes, but saves costs across the entire community. Finally, we leverage data and insights from our technology and care coordination to inform population health research. Through partnerships with research institutions and universities, we’re working to gain a richer understanding of how and why a values-based model of care works. This research is ensuring our work is not just evidence-based, but evidence-generating, so that we can make the case for values-based healthcare in all communities.
