Mission Statement
MHP is an organization that employs parents successfully navigating the system of care for their children. This innovative staffing model has allowed MHP to forge a unique connection with families in need based on experience, empathy and trust. The Medical Home Plus model is successful because not only do our employees have professional skills, but most importantly they are experts in the family aspect of what it means to support a child with special health care needs. We don't just "talk the talk", but are also "walking the walk".
About This Cause
Medical Home Plus is dedicated to empowering children with special needs and their families by strengthening their pillars of support through education, prevention, resource coordination and advocacy. Founded by pediatricians to help parents navigate medical and educational arenas, MHP utilizes the “medical home” model holding that strengthening and empowering families is the best way to serve special needs children. MHP connects parents with Resource Coordinators (also parents of special needs children) who help on average 200 families each year with support, information, and advice. MHP proves helpful in making decisions about health care, useful for finding additional services, and boosted their confidence to advocate for their child.
