Mission Statement
Sentry Living Solutions shall provide to adults with developmental disabilities the knowledge and skills training necessary to acquire and maintain a self-sufficient and independent life-style. Sentry Living Solutions intends to provide and coordinate support services, referred to as Independent Living Services (ILS), with a focus on fundamental skills training for adults who have achieved basic self-help capabilities. Sentry Living Solutions shall develop an individually designed support services plan for adults who have some self-help abilities, but find it necessary to have supervision or assistance in meeting their personal needs.
About This Cause
Objective: Our objective is the empowerment of the client (also referred to as the consumer) through knowledge and skills by the presentation of a clear outline of each service specifically designed for the client so they may become as independent as possible. Goals: Sentry Living Solutions’ independent living services (ILS) takes an holistic approach coupled with exceptional service and individual training to ensure the client lives well, safe, and happy. INDEPENDENT LIVING SERVICES A program plan curriculum in training/instruction in the following areas: Cooking Cleaning Shopping (providing information about products/comparisons) Menu planning Meal preparation Money management (check cashing and purchasing) Use of public transportation Personal health and hygiene Self advocacy Independent recreation and participation in natural environments Use of medical and dental services as well as other community resources Community resources awareness such as police, fire, emergency help Home and community safety Reflect activities similar to those of non-disabled adults. Developing and maintaining consumer’s functional skills. All of the above are critical focus areas determined by the client’s needs and the establishment of an Individual Service Plan (ISP). PROGRAM DESCRIPTION PHILOSOPHY OF SERVICE Sentry Living Solutions’ independent living services (ILS) program is designed on a home and community based instruction model in which education and activities are immersed as much as possible with a consumer to staffing ratio of 1:1. Sentry Living Solutions focuses on self-determination skills, vocational skills, independent living skills, and community integration. Consumers are offered 30 service hours per month for a total of 360 hours annually. The days and times of service will be developed between Sentry Living Solutions Coordinators and the consumer and/or families. The ILS program will not only support the client’s independent living skills development but will also support their pre-employment training and their leadership skills development. The Sentry Living Solutions’ ILS/Adult transition program empowers consumers through a process of measurable, evidence-based instruction involving community experiences. The community integrated experiences provide the opportunities necessary for consumers to practice skills, gain confidence, and build self-esteem and self-efficiency. Sentry Living Solutions is a person-centered program guided by consumer choices and interests. Sentry Living Solutions provides a safe environment so that consumers can engage in new experiences with confidence and success as they work to find their special place in the world. Sentry Living Solutions program believes empowerment is nurtured through community involvement. Our program will establish collaborative relationships with a variety of agencies and non-profit organizations working with diverse populations. COMMUNITY BASED AND HOME INSTRUCTION Sentry Living Solutions community based instruction is an educational instruction tool that takes place in typical community spaces. These environments are quite different from the school campus and classroom. Sentry Living Solutions’ instruction provides consumers with the skills commonly needed to function as productive adults in real life settings. Under the supervision of Sentry Living Solutions’ staff and aides our community based instruction prepares consumers in a less restrictive environment for a successful transition to living independently and thereby enhances their quality of life. Of necessity, a planned day could be changed due to weather conditions, transportation issues or any other unforeseeable problems. Home instruction is also utilized to provide learning opportunities in a familiar environment. It is understood that once skills are presented, a majority of instruction will be in the community. For example, a consumer can learn about bus and Bart routes in the home, but they will not be able to utilize and master those skills until the skill is used in the community. Program schedules are individualized and are organized and developed based on each consumer’s Person-Centered Plan (ISP). Sentry Living Solutions’ community-based instruction may occur in various settings throughout the consumer’s local area including restaurants, the local library, gyms/stores, community colleges or volunteer work sites. The instructional meetings will take place in the consumer’s home and community and are based on the consumer’s needs and preference. Transportation to and from community locations is via the following systems: Para Transit, taxi, bicycle, BART (Bay Area Rapid Transit), AC Transit bus, public transportation, or walking. PERSON CENTERED PROGRAMMING Sentry Living Solutions’ consumer service plan (ISP) is driven by individual consumer choices, incorporating the client’s vision of their future to guide and focus the direction of their services. (Due to HIPPA and privacy policies a sample ISP is provided with fictitious names.) Sentry Living Solutions consumers are respected as adults with hopes, dreams, boundaries and individual rights. Sentry Living Solutions interaction with consumers emphasizes individual strengths and capabilities through affirmation and encouragement. EMPOWERMENT AND PERSONAL RESPONSIBILITY Sentry Living Solutions maintains a safe, nonjudgmental space where consumers gain confidence in their own ability to make decisions about fundamental aspects of their lives. Sentry Living Solutions clients learn to be accountable for their decisions, choices and actions. Sentry Living Solutions teaches respect of personal space, property and privacy. SELF-DETERMINATION SKILLS Sentry Living Solutions promotes consumer independence and empowerment by incorporating the following seven components across all learning environments: Choice-making Problem-solving Decision-making Goal setting and attainment Self-regulation Self-advocacy Self-awareness Self-efficacy PROGRAM DESIGN CHANGES When revisions are made to the program design which constitutes a change in the type of services provided, Sentry Living Solutions will give 30 days notice to the consumer and their authorized representatives and submit to the tendering regional center, a revised program design. A revision in program design is presented when any of the following elements of the program design occur: Location in which student training occurs Curriculum training components Existing approved staff ration Approved service code Entrance and exit criteria Request for increase capacity regarding maximum enrollment
