Mission Statement
The mission of Rehab and Beyond is to provide free therapy to low-income, uninsured, or underinsured stroke and brain injury survivors in the state of Georgia, regardless of circumstances.
About This Cause
Using an integrated wellness approach, Rehab and Beyond combines physical and cognitive rehabilitation therapies, medical exercise, motivation and physician backed health coaching to improve the quality of life for stroke and brain injury survivors. These services are provided at a designated medical facility by licensed and certified providers at no cost. Our program bridges the recovery gap and transitions participants from patient to self-dependence. Started in 2016 by providing therapeutic services to 6 stroke survivors and 2 brain injury survivors, while volunteering rehab and health coaching services in stroke and brain injury support groups hosted at medical facilities such as Emory Rehabilitation Hospital, Northside Medical center, and The VA Hospital. Rehab and Beyond has grown to be one of the largest community advocates for underserved stroke and brain injury survivors. The organization is staffed by 8 employees and supported by over 100 regular volunteers. Since its founding as an independent non–profit in 2018, Rehab and Beyond has served 750 stroke and brain injury survivors, 250 caregivers, including transitioning 20 survivors from wheelchair to no ambulatory device usage. Rehab and Beyond is organized as a nonprofit in the State of Georgia and is a recognized 501(c)(3). Rehab and Beyond recognizes the need for integrated and closely coordinated services for this vulnerable population. We have active partnerships with more than 10 health and medical organizations providing critical services like neurological rehabilitation, physical therapy, occupational therapy, recreational therapy, speech therapy, rehab training, and physician-backed health coaching. 4 times per year, Rehab and Beyond hosts an event in Georgia that provides almost 250 volunteers to assist in providing free therapy to the low-income, uninsured or underinsured stroke and brain injury surviving community. PHILOSOPHY & APPROACH Our programs and services have been expanded and enhanced to reflect a self-dependence model. Under this model, Rehab and Beyond first addresses the basic human need for neurological health and self-dependence that people who are stroke or brain injury survivors are experiencing, then provides a wide array of professional services to support the individual and address obstacles to self-dependence. This model recognizes that suffering from a stroke or brain injury is not a minor concern and seeks to provide support services to stroke and brain injury survivors beyond traditional methods. Rehab and Beyond provides an initial evaluation and assessment of current conditions. A questionnaire containing goals and aspirations is given to the stroke or brain injury survivor to complete. A proper therapy treatment plan, based on the needs of the survivor is implemented. During the extended therapy treatment plan, the survivors and caregivers are educated on day to day activities(outside of the therapy sessions) to perform that will reinforce the gains that are made in therapy. Survivors and caregivers are introduced to additional resources such as support groups that meet monthly, equipment resources that provide needed medical equipment such as walkers, wheelchairs, canes, beds, prosthetics and orthotics. Rehab and Beyond will provide access to a community/ free therapy day that is held quarterly, at our clinic by licensed therapists in all areas such as physical therapy, occupational therapy, speech therapy, recreational therapy, and rehab training. At this event, low-income uninsured or underinsured stroke and brain injury survivors receive additional free therapy and education of care for survivors and caregivers by providing different fields of rehabilitation. A post treatment progress report and questionnaire is implemented to ensure benchmarks. STATEMENT OF NEED Stroke and brain injury rehabilitation can be very costly to both survivor and caregiver, but is the key component to ushering the survivor back to self-dependence. According to the National Stroke Association, Stroke is the leading cause of adult disability in the United States. Nearly 800,000 Strokes occur each year. The Center for Disease Control (CDC) estimates that Stroke rehabilitation costs the U.S. over 34 billion dollars. Acute care and rehabilitation of brain injury patients costs the U.S. about 10 billion dollars per year. Through Rehab and Beyond’s Rehab training scholarship program(RTS), low-income, uninsured, or underinsured stroke and brain injury survivors receive free therapeutic services in the form of rehab training, physical therapy, speech therapy, occupational therapy, and/or recreational therapy. In addition to being awarded the RTS program, the recipient and their caregiver/loved ones are automatically included in our total “community outreach”. With our existing partnerships and relationships with medical facilities, organizations, and individuals, Rehab and beyond also provides access to resources, such as support groups, community events, post-incident education, and post-incident resources, for needed medical devices and equipment. Rehab and Beyond believe that providing needed therapeutic services is not only the key to ushering each survivor back to self-dependence, but also improving and reducing the workload that is transferred to the loved ones that care for the survivor on a daily basis. Rehab and beyond also believes that providing needed professional resources, in addition to the RTS program, can reduce the continuous search on how best to treat and care for an individual who has suffered a stroke or brain injury. Our partnering hospital, Emory Rehabilitation Hospital provides licensed volunteer therapists that facilitate care and education, in addition, to support groups that broaden the community of each survivor. Most of the services are provided at scheduled times throughout the week at our facility located in Tucker, Ga. Typically, a survivor will be provided therapeutic services for 2.5 months and Rehab and Beyond will continue to provide education and resources after the extended treatment plan has been implemented. Accountability In our Rehab training scholarship program (RTS), the expectation is that 80% of participants will obtain self-dependance abilities within 2 months. We also recognize that participation in the program imparts physical, cognitive, and educational skills that will benefit participants throughout their lives, so we expect participation at 90% of the scheduled therapy sessions. Our case manager takes attendance at each session and logs the attendance against the treatment plan developed specifically for each stroke and/or brain injury survivor. If the patient is falling behind in their participation, the case manager and/or director will reach out to the patient to develop a plan to get them back on track. Qualitative Feedback from patients In addition to having a documented evaluation performed on the patient, the patient will receive a start-up a questionnaire at the beginning of the treatment plan, to address the area of improvement desired. Following the final therapy session, which includes a post-therapy progress report, the patient will receive a post-treatment survey on their overall results and satisfaction with the program, so that we can assess qualitative feedback for continued growth and success in our program. Sustainability Rehab and Beyond are committed to the diversification of its revenue streams and funding sources. Over the past 3 years, Rehab and Beyond have collaborated with 12 funders to support its array of programs and services. Likewise, this therapeutic service program is being developed with the commitment and involvement of 5 different service partners providing various aspects of the program, such as therapy services, rehabilitation/health education, rehabilitation/wellness equipment, and social platforms for stroke and brain injury survivors. These partners are committed to providing these services to low-income, uninsured, or underinsured stroke and brain injury survivors, on an ongoing basis. Finally, Rehab and Beyond have an active fundraising department dedicated to bringing in discretionary revenue that is used to build infrastructure, and where possible hire staff to institutionalize our new programs. Over the last 3 years, these discretionary funds have increased 65%, allowing the organization to hire two new staff. The Rehab and Beyond Board of Directors expects the level of discretionary funding to increase over three years. Partnership & Volunteers Rehab and Beyond has a long history of developing strong relationships with local partners to help fill gaps in services and bring local expertise to the problems of low-income stroke and brain injury survivors attaining therapeutic services. Local partnering hospitals such as Emory, Shepherd Center, Northside, Grady, and Clarkston Medical center provide 40 to 50 licensed and certified providers that volunteer their services by providing therapy and post-incident education to the needed survivors. The Dr. Sears wellness institute provides physician-backed health education, while the Trellis Horticultural Therapy Alliance offers social platforms for all participants to continue their recovery, beyond the initial program. Fodac and Anytime Fitness provide needed equipment to our facility and survivors in need to further and enhance the rehabilitative care. Finally, Rehab and Beyond is an active member of several stroke and brain injury surviving support groups, where we provide rehabilitative education, free therapy, and host events. Board of Directors Rehab and Beyond is overseen by a 9-person Board of Directors that is actively engaged in strategic planning, relationship development, and community involvement. The board is made up of five men and four women. Four members are African-American, the rest are Caucasian. Six are leaders in their respective fields and organizations representing healthcare and social services. One member-at-large has a daughter that is part of the Rehab and Beyond program.
