Mission Statement
The mission of Sunrise Community Health (Sunrise) is to provide affordable access to quality health care for all. Our vision is that healthy people create healthy communities, and all of us should be able to access the right care, at the right time, for the right cost.
About This Cause
Sunrise Community Health is a non-profit, award-winning federally qualified health center serving Northern Colorado from eleven clinic sites in Greeley, Evans, and Loveland. Our dedicated and talented team of nearly 400 offers medical, dental, behavioral health, and vision care as well as laboratory, X-ray, pharmacy, social work, health education, WIC, and care management services to 41,000 people each year. As a Community and Migrant Health Center, nearly 90% of our patients live at or below 200% of the federal poverty level; 53% have Medicaid, 27% are uninsured, 13% have private insurance, and 7% have Medicare. Sunrise also serves a diverse patient population, including 72% from racial and ethnic minorities (63% Hispanic/Latino) and 43% who are best served in a language other than English. Sunrise is dedicated to developing the next generation of health care workforce. As a recognized leader in education health center activities, we offer 20,000 hours of education each year to students of all disciplines – physicians, dentists, physician assistants, nurses and nurse practitioners, medical assistants, social workers, and high school students who are just beginning to explore healthcare as a possible career. Sunrise strategically designs community collaborations to connect resources and care to serve the whole person in a coordinated, efficient, and economical manner. Our innovative partnerships reach across the spectrum of care to include behavioral health, public health, hospital systems, specialists, and homeless services, as well as community-based care navigation and care management.
