Mission Statement
Since 1950, Wesley Community and Health Centers (Wesley) has provided community programs and services to low-income, primarily Hispanic families residing in south - central Phoenix. The mission of Wesley, "Together We Empower Positive Change" extended to a second location in west Phoenix in 2014 with the aquisition of Golden Gate Community Center. Wesley now consists of two community centers and two Federally Qualified Health Centers (FQHC) in two locations (south - central and west Phoenix). Wesley's integrated health care model serves the community with safety-net primary care services and community center activities such as community gardens, two full size indoor basketball courts, after-school and summer programing for children ages five to15, education classes (ESL, GED, nutrition, exercise), a computer lab and two soon to be fully licensed kitchens. Programs and services at both Wesley locations continue to expand along with the number of new health center patients.
About This Cause
Since 1950, Wesley Community and Health Centers (Wesley) has provided community services in Phoenix to potentially at-risk families. Wesley is unique, having the prestigious designation of being a Federally Qualified Health Center (FQHC) as well as a full service community center. To receive FQHC designation, a health center must be located in a Federally Identified Poverty Area and serve low-income individuals [100 percent or below the Federal Poverty Limit (FPL) or $24,600 for a family of four]. Wesley provides safety-net primary care and community center programs and activities among 7,000 low income, uninsured or underinsured children and adults in two Phoenix communities (south-central and west Phoenix). Wesley’s family health services include (but are not limited to), cancer screenings, child-wellness exams, chronic disease management, family planning, immunizations, OB/GYN, prenatal care, urgent care, and well woman visits. More than 70 percent of Wesley's population is uninsured, making Wesley the smallest Phoenix area FQHC serving the largest percentage of uninsured patients. In addition to primary healthcare services, Wesley provides key community programs and activities through; two full-size, multi-purpose, indoor gyms; community gardens; after-school and summer care programs for youth (ages five to 15); adult education classes (Adult Literacy, ESL, GED, nutrition); citizenship classes; exercise classes (Zumba); and two- soon to be fully licensed community kitchens. Wesley's services are available to anyone regardless of their ability to pay. Health center services are provided at reduced rates, and/or sliding-fee scale, based on family size and income. Community center programs and activities are provided for a nominal fee or extremely reduced rate to allow for increased community participation. Wesley's integrated wellness model is a holistic approach that combines primary-care services with community center programs and activities to complement one another and improve the health of the community.
