About This Cause
ACC Care Center (formerly ACC Nursing Home) ACC Care Center was established in 1987 as a 99-bed state licensed skilled nursing facility located at 7801 Rush River Drive in Sacramento, California. Services are designed to meet the needs of short-stay rehabilitation patients and long-stay residents who make ACC Care Center their home. ACC’s focus on quality care and clinical systems has earned it a 5-star rating from the Center for Medicare and Medicaid Services from July 2013 to the present. The Silver Award from the American Health Care Association (AHCA) National Quality Award Program, modeled after the Malcolm Baldridge Quality Award, is among the most recent recognitions achieved. ACC Senior Services Programs ACC Senior Services Programs, since 2002, include a Lifelong Learning & Wellness Program (offering 50 class sessions a week), ACC Rides Transportation Services (providing 3,000+ one way rides a month). In the last 9 years Bridge to Healthy Families Caregiver Support Program (visiting over 50 families a year), Utilities Education & Assistance, and, most recently, Senior Community Service & Employment Program (enabling job training for 100 low income seniors) have been organized to respond to address the needs of older adults and their family caregivers. ACC Greenhaven Terrace On October 4, 2007, ACC acquired a 166 unit independent living apartment community for senior adults located at 1180 Corporate Way. It is now called ACC Greenhaven Terrace. The three-story building, constructed in 2000, offers one and two bedroom apartments with fully equipped kitchens, central heat and air conditioning, walk-in closets, and a private patios or balconies. The pool, fitness room, library, computer room, and community room offer opportunities for shared activity and socialization. Apartments are offered at competitive market rates. In April 2014, ACC’s Assisted Living was licensed by the State Department of Social Services as a Residential Care Facility for Elderly. We were able to transform 27 independent living apartments into 24 one-bedroom apartments for assisted living, a dining room, an activity room, and beauty salon. The added ‘level of care’ spurred a greater effort to work with residents over time as their care and support service needs change, to provide a better continuity of care, and to emphasize a larger sense of community for residents. Independent and Assisted Living residents, for instance are more commonly joining Lifelong Learning and Wellness Classes, participating as part of a ukulele or vocal group. ACC fosters continued effort to move beyond service silos and level of care barriers to enable care givers and care receivers to experience a more seamless use of long term services and supports over a remaining two, ten, or twenty years. ACC Maple Tree Village In February of 2020, ACC welcomed the first residents to it's largest community to date, ACC Maple Tree Village. Offering 69 apartments for Assisted Living and 30 dedicated apartments for Memory Care support. ACC Maple Tree Village features a state-of-the-art design with vast community spaces, "neighborhood" meeting areas and fully accessible amenities. From the sunny courtyards to the open dining room, ACC Maple Tree Village offer residents a home-like setting.
