Mission Statement

Our mission is to create opportunities where people can learn, grow, and age independently with dignity in community.

About This Cause

Choice in Aging, formerly Rehabilitation Services of Northern California (RSNC), was established in 1949 as a private, nonprofit 501(c)(3) public benefit corporation. Choice in Aging has centers in Contra Costa County and serves more than six hundred people each year. The Choice in Aging Board of Directors are dedicated community volunteers who provide guidance, support, and their expertise to the organization. Our programs include Adult Day Health Care services for frail adults, elders, and persons with Alzheimer’s disease and related dementias. This program is designed for loved ones who can engage in a safe and supportive environment during the day, while still living at home with their family members. Adult Day Health Care services are licensed by the California Department of Public Health and certified by the California Department of Aging. We also offer the Multipurpose Senior Services Program (MSSP), a complex case management program for frail elders to assist them in their homes so they can remain living independently. Our California Community Transitions project allows us to assist long-term skilled nursing facility residents in moving back into community living. We fondly refer to it as our jail break program! Most recently we brought a Montessori preschool into our mix of services which provides the first intergenerational programming opportunities for preschool students and frail elders in Pleasant Hill. We are also proud to offer information and referral resources to the local community, as well as support services to caregivers and family members of frail elders and persons with Alzheimer’s and related dementias.
