Mission Statement
Our mission is to support independent living for elders and adults with health challenges through a day health program that optimizes the health and well-being of San Francisco's diverse communities. We help seniors and disabled adults to delay or avoid costly hospitalizations and/or entry into skilled nursing so that they can continue living in the place they call home.
About This Cause
Founded in 1983, SteppingStone is San Francisco's largest independent non-profit provider of Adult Day Health Care (ADHC). Our primary objective is to help clients improve their health and quality of life, and to maintain their independence in their community by providing a cost effective and comprehensive array of healthcare services under one roof. We promote health equity for the City's underserved population of low- income seniors and disabled adults by increasing access to health and social services. We are leaders in advocating for seniors' rights and their care. Our staff reflects the diversity of our clients and our board is comprised of community members and caregivers. Our program model is a time tested, community-based approach that prevents or delays unnecessary placement into hospitals, emergency rooms and/or nursing homes. Everyday, we transport low-income seniors and disabled adults from throughout the City to our four centers which are strategically co-located with affordable senior housing. Many of these people are isolated with multiple health challenges. By providing skilled nursing; social work services; medication management; physical, occupational, & recreational therapies; meals; personal care; and opportunities to form meaningful connections, we delay and often prevent costly hospital stays, emergency room visits and/ or nursing home admissions. Each year we provide critical health and rehabilitative services to over 350 low-income frail seniors and disabled adults, translating into 50,000+ client service days.
