Since 1975, Serra Center has been committed to providing compassionate care for individuals with developmental disabilities. We currently serve more than 70 men and women in our community-based Residential Care Homes, Independent Living and Supported Living Services. The individuals served live in more than 65 traditional and accessible homes and apartments in residential neighborhoods throughout Alameda County.

About This Cause

Serra Residential Center is a grass-roots non-profit organization founded in 1975 by a group of parents seeking an alternative for their adult children to the institutional care model then being provided for persons with developmental disabilities. The agency was initially designed as a campus-based peer living center aimed at integrating adults with developmental disabilities into the community. As the trend for services in California began moving away from large facility-based care towards small, individualized supported living arrangements, in 1996 Serra Center sold its campus, becoming fully inclusive with the community. With Serra Center services, persons with developmental disabilities are empowered to live the life of their choice, as they would without a disability. Our mission is simple; we provide daily living skills support and training to adults with developmental disabilities. Serra Center accomplishes this mission through our three living skills program: Residential Care Homes, Supported Living Services and Independent Living Services; and Counseling Services. The services offer varying levels of support and care, to meet the unique care needs of each individual and their families. Residential Care Homes, which are licensed by the California Department of Public Health, provide 24-hour, long-term care, supervision and support to individuals who have a recurring but intermittent need for skilled nursing and behavioral modification services. The four homes owned by Sera Center, are single-family residential homes located in neighborhood communities throughout Fremont, Ca. The homes offer a familial environment in which each of the 6 individual’s specific and intensive care needs are met, while providing opportunities for employment and educational training, recreation, socialization and community integration. The homes are funded through Medi-Cal. Supported Living Services consist of a broad range of services, providing for a significantly high volume of support and training to individuals living in homes or apartments throughout the communities of Alameda County. These services help individuals work toward their long-range personal goals, as well as exercise meaningful choice and control in their daily lives. Services are designed around and the needs and desires of each individual and can include more intensive support, such as the use of adaptive equipment and personal attendant care. Some examples of SLS support are: selecting and moving into a home, choosing personal attendants and housemates, personal care, common daily activities and emergencies, participating in community life and money management. Supported Living Services are offered for as long and as often as needed, with the flexibility to meet an individual’s changing needs over time, and without regard to the level of disability. Supported Living Services work with the individual to establish and maintain maximum control over the services they receive, within a safe and stable environment of their choice. Serra Center’s Supported Living Services are funded through the California Department of Developmental Services (DDS) and regulated by the Regional Center of the East Bay (RCEB). Independent Living Services differs from Supported Living Services based on the frequency and level of support provided. Individuals live in homes or apartments throughout the communities of Alameda County, with daily living skills training and support focusing on the individual leading the most self-managed life possible in the least restrictive environment. The number of training hours and the areas of training are determined based on an individual’s assessed need and personal choice. It is expected that over time, an individual’s need for services may decrease, at which time on-going support is provided at a lesser frequency. Individual’s receiving Independent Living Services typically receive up to 40 hours per month of support and training. Some examples of training areas are: household maintenance, personal safety, money management, comparison shopping, nutrition and cooking. Serra Center’s Independent Living Services are funded through the California Department of Developmental Services (DDS) and regulated by the Regional Center of the East Bay (RCEB). Counseling Services help individual’s living with developmental disabilities and various forms of mental health issues to better understand their condition so they, their families and caregivers may more effectively manage symptoms. Serra Center’s services are designed to help support individuals with personal goals and growth, positive decision making, strengthening social skills and relationships, identifying choices and promoting independence. We offer individual therapy, family therapy, couples therapy, parenting support and groups. An intern based program, Serra Center is committed to providing community training to therapists so that they feel competent to interact effectively with people living with a developmental disability (or with dual-diagnosis), to develop a self-sustaining network of mental health resources to serve our community into the future. Services are offered on a sliding scale basis, or through insurance when applicable. People living with developmental disabilities require services that are uniquely designed to meet the complex, special and changing needs of each individual served. By providing high quality, individualized and consistent supports, a strong sense of community is developed. Individuals are empowered to meet personal goals, grow, make positive decisions, identify choices, and strengthen social skills and relationships. Serra Center’s philosophy to serve the whole person versus the disability, leads to greater emotional well-being, improved self-esteem and endless opportunities.
