On Lok / On Lok Lifeways provide comprehensive care for elders, including complete medical care, social services, meals, transportation… enabling frail seniors to remain in their home and community for as long as their health permits. This PACE center is a caring alternative to nursing homes.

About This Cause

In October, 1971 On Lok was founded to offer a caring alternative to nursing homes, enabling frail seniors to remain in their homes and communities. On Lok provides care and support so that seniors can age in place, enjoying the comforts of home while receiving comprehensive care. Compared to their peers, seniors served by On Lok Lifeways receive a more robust package of health and social services, and as a result, exhibit better health. On Lok Lifeways stresses the importance of an individualized care plan created by a team of doctors, nutritionists, social workers, physical therapists, recreational therapists, home care staff and other professionals. Upon joining On Lok Lifeways seniors often get a new lease on life, because they are relieved of the challenges of daily life and the gaps of fragmented care. As an On Lok Lifeways social worker expressed it, “On Lok Lifeways’ all-inclusive program really is the ideal way for seniors to receive care.” In 1979, On Lok developed On Lok’s model into PACE (Program of All-inclusive Care for Elders). This replicable model for long term care utilizes interdisciplinary teams, coordinated and preventative care to provide all medical care and social services. In 1997, On Lok campaigned for the permanent provider status for PACE programs (designating PACE as a benefit of Medicare and Medicaid) under the federal Balanced Budget Act of 1997. This laid the groundwork for On Lok’s model to be adopted by agencies throughout the nation. On Lok’s Fremont Center was launched in 2002, expanding our services to southern Alameda County. In 2009, On Lok Lifeways opened the San Jose Center, our first center in Santa Clara County. Today, On Lok serves approximately 1,100 seniors at ten centers throughout the Bay Area. Almost all of the seniors that On Lok Lifeways serves are extremely low-income with a median income of $812 per month. Most On Lok seniors (94%) are Medicare and Medicaid eligible. The participants served by On Lok are an average age of 84 years, and meet California state eligibility requirements for nursing home placement. Every one of our participants has functional impairments and 60% have some form of dementia. The seniors are diverse, and many are immigrants: 50% are of Chinese descent, 15% are of other Asian backgrounds, 17% are Caucasian, 12% are Latino and 5% are African-American. For 40 years On Lok has been committed to meeting the health care needs of the frail elderly in the Bay Area. Our unique approach serves older adults residing in San Francisco, Fremont and San Jose, who are 55 or older and experience multiple medical problems that inhibit their ability to live unassisted. Created the PACE (Program of All-inclusive Care for the Elderly) model of eldercare, which has been adopted by 79 programs in 30 states and helped found the National PACE Association and CalPACE Association to assist agencies in adopting PACE and advocate for PACE on the state and national level. 3. On Lok has been honored with many prestigious awards, including the Brookdale Award for Best Practices in Human Resources, The American Society on Aging Leadership Award, and the Pinnacle Award for Innovations in Medical Management by the College of Physicians’ Executives. Last year, On Lok received the Justin Ford Kimball Innovators Award from the American Hospital Association recognizing On Lok’s outstanding contributions to health care delivery and financing. 4. Working with rural agencies throughout California to adapt the PACE model for the unique challenges of rural seniors. 5. Was the first long-term eldercare provider to adopt Electronic Medical Records (in 1996) to improve coordination of care and ensure the privacy of our seniors. We have recently developed and launched “PACELink” a new, sophisticated EMR system with applications for long-term care facilities nationwide. 6. Operate the nationally lauded 30th Street Senior Center, engaging over 5,000 low-income and diverse seniors in a variety of activities to improve their health and support their independence. 30th Street’s innovative programs include “Always Active” a citywide program of health promotion for seniors, brain fitness and creative arts programs, and other vital services. 7. In San Francisco, a city with a dire shortage of affordable senior housing, On Lok provides HUD 202 housing for 73 frail elders at On Lok House; and offers housing at two centers, for 89 low-income, frail elders.
