About This Cause
Home of Guiding Hands (HGH) has been committed to providing quality community support, critical housing and life skills training for children, men and women with developmental and intellectual disabilities for 56 years. Initiated in 1967 by a group of ambitious East County leaders, we have evolved from a single 14-acre campus to a provider of a full spectrum of services to more than 4,500 infants, adolescents, and adults throughout San Diego and Imperial Counties. The basis for what we do is the belief that individuals can thrive in a setting that offers a more fulfilling lifestyle, provides individualized attention, stresses increased opportunities for self-sufficiency, and encourages greater community integration and access. To be eligible for services, individuals must have a primary diagnosis of a developmental and/or intellectual disability such as autism, cerebral palsy, epilepsy, etc. Referrals originate from the San Diego Regional Center which acts as the entry point for services and supports for persons with developmental and intellectual disabilities. HGH fully embraces the philosophy of the whole person, respecting familial, cultural and environmental influences which contribute to the unique personality characteristics and individual skills of each person. We emphasize a collaborative partnership and maintain a commitment to the use of best practices and when providing services to persons with developmental disabilities. We believe that all individuals should have access to programs that enable the individual to achieve optimal success and outcomes. Each individual will be empowered to exercise choice and personal rights. Person-directed services are a constant pursuit. HGH believes that all people have the right to be treated with dignity and respect. Our Primary Programs Include: •Residential Services: Throughout our 31 homes, we provide support to children, adolescents, and adults with intellectual and developmental disabilities to include autism, cerebral palsy, and epilepsy. Individuals range in age from 8 to 84 and live in comfortable, friendly environments with 24-hour care and varying levels of individualized support and training. •Adult Family Home Agency: The AFHA program is our adult foster program and is designed for individuals who wish to live with a family. The individual is integrated into family life, receives support from the family, and is involved in the everyday happenings and joys of a home. •Community Living Program (CLP): We support and encourage the greatest level of independence for our clients who desire to live on their own. Each week a trained counselor provides one-on-one assistance in the areas of money management, meal preparation and planning, shopping and accessing community resources in efforts to best maximize their self-sufficiency. •Early Childhood Development Services (ECDS): A dual purpose program which provides therapeutic and educational early intervention to young children, from birth to three years of age, who are at risk of delay or have been diagnosed with a developmental disability. Under the umbrella of ECDS lies our Infant Development Assessment (IDA) program. We evaluate children from four months to three years of age in order to establish eligibility for CA Early Start services. Assessments are conducted in the home. •Respite Services: We provide in-home support to enable a family/caregiver to feel comfortable taking a break from the intensive supervision and care of their loved one. We honor and respect the physical and emotional energy it takes to preserve the commitment to keep their family member in their home. •Tailored Day Programs: Meaningful relationships are the foundation of a happy life. Through our Tailored Day program, we help individuals with disabilities build quality relationships by assisting them to access community activities where they will meet others with shared interests. Through this process, program participants also develop their self-advocacy skills by increasing control over their lives and making choices about the way they want to spend their time. •Transportation: We offer specialized transportation services for individuals with intellectual and developmental disabilities, many of whom experience significant physical and/or medical challenges. Our fleet of adapted vans transports people throughout San Diego County. This service enables clients to connect with others in their community in a safe and effective manner. •Life Planning: Life Planning is a workshop series to help people develop a life plan for a family member with a disability. We educate families about available resources to ensure quality of life throughout an individual’s lifetime. Families gain reassurance and peace of mind from knowing they have plans to ensure the best life possible for their loved ones. We help identify future needs and consider what should be in place when families can no longer provide care. Our Universe: Home of Guiding Hands is unique compared to many organizations, due to the versatility of our programs and services available to our clients. We provide a continuum of care starting at birth through the end stages of life. We offer customized individual care and support based on the strengths and abilities of each person. The defining philosophy that drives our organization is that all individuals deserve access to the services that enable them to achieve optimal success in life. Our services are person-directed. Respect and dignity guide our programming encouraging individuals to dream and pursue fulfillment. We accomplish this by offering a variety of choices so that people can direct their own lives by choosing programs that best suits their individual needs and wishes. How do we compare to other agencies: Many other providers immediately assess and inform clients what services they need. They are often times trapped in these services with no options. Choices are not guaranteed and enjoyed. The services focus on what they are unable to do in life based on that disability. Other organizations narrowly focus their efforts and programs based on criteria such as age, specific disability, or specific programs such as day programs, vocational services, residential services, transportation, respite services, independent living services etc. Home of Guiding Hands is proud to provide and customize all of these services as one vendor. We are able to then see these individuals grow and gain independence and move them along in the services and programs they desire and need. It is an approach where re-evaluation is constant and growth is expected and fostered throughout the individual’s life. Outcomes & Measurements We support a vulnerable population of individuals with developmental and intellectual disabilities whom would not survive without services like that of HGH. Clients range in age from 8 to 85, we consider our efforts successful by ensuring the health and well-being of each individual is addressed. This goes beyond scheduled meds passes, basic hygiene care and meal planning. The physical, spiritual, and emotional health is considered in the homes we provide, by the activities we plan and the life lessons we teach every day. Our direct service professionals range in credentials from CNAs to M.D’s; each following a carefully planned individualized care program for every client. These requirements are set forth by the Regional Center, and regulated by the State of California division of developmental services, health and human services, the local health department. In todays climate, we are also measuring our success by the negative Covid tests we receive for our care professionals and our residential clients.
