About This Cause

From humble beginnings we have expanded services to meet the needs of our community to include skilled nursing/rehab, hospice care, intermediate/adult residential care, caregiver support, senior day care, Meals to Go, and several in-home services designed to assist seniors age 65 years old and over who require assistance or supervision with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). For over 125 years, PCH has been a leader in providing quality specialized care programs and caregiver support both in our facility and in homes across Oahu. A 501(c)3 nonprofit, Federal ID#99-0073521 . Providing quality care for Hawaii's seniors since 1896. Palolo Chinese Home’s mission is to educate, support, and care for all seniors and their families by providing a continuum of quality care in a culturally sensitive family setting. Our vision is to become a state-of-the-art, accredited, continuing care retirement community with both on-site and off-site activities serving a wide range of multi-ethnic elders in a culturally sensitive environment on a beautiful campus in Hawaii. Palolo Chinese Home’s (PCH) core programs that include skilled nursing/rehab, hospice care, intermediate/adult residential care, caregiver support, senior day care, Meals to Go, and in-home services called the Palolo Wellness Program (PWP) address the Health and Wellness sector in the CHANGE framework and aligns perfectly with the sub-sector under Improvements in Social Determinants of Health. PCH engages the expertise of its interdisciplinary team made up of physicians, geriatricians, registered nurses, physical, occupational and speech therapists, social workers, chaplains and financial assistance counselors. PCH works collaboratively with a wide array of specialized service providers that form our unmatched continuum of care. To ensure that our programs and services reflect the needs and desires of our population, PCH conducts regular resident/family satisfaction surveys and council meetings to determine appropriate strategies in accordance with established evidence-based best practices on guidance from the DOH, DHS, State Health Planning & Development Agency (SHPDA), and the Centers for Medicare and Medicaid Services (CMS).
